Opinions on what to do to 100% repair my damaged fender?

Brett, I think you got very lucky with this solution. Looks pretty much perfect. I can tell you with certainty that 9 out of 10 PDR guys in my area would have said "no way" or "I don't have the time to work on something like that". You obviously found the needle in a haystack and I'm sure you must be 110% pleased with the outcome.
 
Can you see anything left over on the crease?

If you don't mind me asking, how much does a repair like that cost?

I'm always amazed by what those guys can do. I had this one repaired to 99%. Much better than doing a new fender and paint matching/fitting IMO.

I narrowed it down to 3 PDR specialists in my area. The first two turned me down from the pictures, but this one was confident he could repair it 95%+. I was nervous so I told him to work on the first half of the fender so I can see if I want to proceed. When I saw what a great job he did on the largest dents it was evident the rest would be a cake walk for him. He really is the best I've seen, period.
 
I'd say it's well worth the price and you made the right choice just so long as you're eye doesn't catch it when you walk by. I'm obsesive this way and little things really tend to bother me.

Is there still a tiny bit of damage still visible at an angle? Yes. Is it worth not having to repaint 3 panels? Absolutely.
