Gift Certificate Template

Does anyone have a template that I can use for a gift certificate, my lapytop is currently being repaired and I can't get to my regular one. I know this is odd to ask but I am in a jam and have someone asking me for 3 and dont want to lose out.

http://ubrf.ddetail.com/gc.pub It is my old template, but it is all i have readily available to send you on-line. You will need to tweak it, but it should do the trick!
