I try and avoid having my customers fill out anything. You shouldn't need a disclaimer, and i would never sign one to have my car cleaned anyhow. I would just ask you for your proof of insurance. The most that i ever have my customers do is sign a vechicle or boat check-in sheet that lists current damage to the vehicle. And they have to sign a copy of the invoice if hey pay cash or check. As far as name and address ph# etc... I get all of that either through the intial e-mail or phone call. It all goes into my computer system in both outlook and quickbooks. It is always important to keep a good customer database. It is hard to market to your current customers if you dont know who they are.
