Leftover water? As in the water you used to wash the car with and stuck the wash mitt back in? If so, this will definetely cause scratches, as you are going to be spraying slightly gritty water then rubbing it into the paint with the claybar.
When claying, use fresh water with car soap or use QD spray as a lubricant.
Oh, and no need to dry the car before claying, imo, just as long as it's completely rinsed.
