a very close call

I was polishing the maxima a few hours ago, trying out the PC and 3m SMR for dark cars. I've been reading the "tricks you've learned thread" and heard (from carguy I believe, though I think it's been said by many) that it leaves a haze so you really have to work it in. I did about a quarter of the hood just fine, to the point where there was a pile of fine powder which had collected after wiping in one direction with a MF. I start the second section and to finish off, put the PC to 6 (I've heard it's necessary to fully polish once it starts to dry), when all of a sudden the yellow pad flies off, while I'm applying very slight pressure. I shut it off right away, but had I been pushing down any more, I would have a nice patch buffed by velcro. Before anyone asks I used enough force when putting the pad on, and checked it after the first application. Needless to say I'm a bit edgy about starting again tomorrow, seeing as this was my first real polishing experience with a buffer (I used p21s cleanser last week at low speed). Has this ever happened to anyone? How can I prevent it from happening again?
