What do you guys use to remove smoke odors from a car?

I usually shampoo all carpets and seats, hit the headliner with woolite and warm water on a mf towel, clean all vinyl and leather and then run a fogger through the AC. I then fog the entire car, let it sit for 45 minutes, open all windows and park it in the sun. Bad cars sometimes take two treatments.
Once you buff black, you never go back
For a little less intense method, I`ve had good luck with Meg`s Odor Removal spray and cig smoke... Just spray on fabric and it leaves kind of a sweet aroma that works pretty well for something that only costs $3... lol

I`ve not had any issues removing smoke odors if the interior is thoroughly cleaned. Steam seems to work really well for releasing odors from fabric.

Also, you`ll want to replace the return-air (inside the cab) filter, if your vehicle had one.
